Trying to track down the creator of the D&D Next fam-designed character sheet

This is a bit of a shot in the dark, but I know a lot of folks on here used to frequent the WotC forums before they were closed down, so I figure it’s worth a try.

Does anyone remember, during the D&D Next Playtest, WotC held a contest where fans could submit character sheet designs, and the winning design would made into an official 5e character sheet? The winning design still comes with the official 5e character sheet PDF file from the D&D website. I’m trying to find the artist who designed that sheet to get permission to use some of the icons from that sheet on a character sheet redesign I’m working on. But the PDF doesn’t have any credit or information about the artist at all, nor does WotC’s website from what I’ve been able to find. If anyone remembers who submitted that design or has any information about how to get into contact with them, it would be much appreciated.
