We have an afflicted wereboar in our party.
I'm not sure how an evil were creature would work out, but (per the MM) neutral ones don't go on the rampage when the player loses control, so its not an issue for us.
Forced changes into animal form - especially as a result of taking damage in combat - can be frustrating for the player. The level adjustment also means it takes a while to gain the next level after being afflicted (at which point you can put ranks in control shape, assuming you are aware of your condition by then!) as you have the dual effect of having a higher effective character level (so get less XP from encounters) but also needing to earn enough XP to work through the level adjustment "levels".
It can also be hard to get the player out of the mindset that "I wander around in hybrid form all the time because its the form with the best powers" - never mind the effect on the locals!
Spellcasters probably won't appreciate lycanthropy's level adjustment, but melee types could become very powerful due to the large strength boost many were creatures get. It takes a very brave BBEG to go toe to toe with our wereboar barbarian ....
