Everything I see is saying that two weapon fighting is only allowed with two weapons held in your hands...for creatures with only two arms/hands.
Allow me to preface by saying that I allow any unarmed strike, not just a monk's, to be performed using any body part. In my view, you do not need to be a dedicated martial artist in order to headbutt someone or knee somebody in the groin.
Beyond that, I'm extrapolating based on the armor spikes entry of the FAQ. Page 41. Relevent snippet:
"...When using armor spikes along with a two-handed weapon, it is usually best to use the two-handed weapon as your primary attack and the armor spikes as the off-hand weapon. You can use the armor spikes as the primary weapon and the two-handed weapon as the off-hand attack, but when you do so, you don't get the benefit of using a light weapon in your off hand."
Flat-out states that you can two-weapon fight using a two-handed weapon and something else, in this case armor spikes. Would seem reasonable that you could use unarmed strikes to similar effect.
