I like the Neverwinter Nights version of toughness, which works a lot like the one Michael Morris described.
I'll write it up here...
TOUGHNESS [GENERAL]
You are tougher than normal.
Benefit: A character with this feat gains one bonus hit point per level. Bonus hit points are gained retroactively when choosing this feat, and bonus hit points continue to be added as the character gains levels.
Special: Unlike the core version of Toughness, this variant version of Toughness may not be taken multiple times.
