What forms do you use while DMing?

The hardest thing for me to keep track of during game play is game time. I use a little ticker sheet to keep track of time.

I also use eTools to generate characters and to print monster stat blocks. With each stat block I draw small circles that represent the number of hit points the monsters has, and then mark them out when damage is taken.

I used to use the Reference Sheets v3.5 (a), but I suspect they might be outdated since the release of D&D 3.5. These were super handy. Does anyone know if these sheets have been updated or are still available?

EDIT: If there is a commercial pdf out there focus on helping DMs during play, like keeping track of combat (i.e HPs, stats, conditions, quick rules summaries, etc) I'd certainly be willing to buy it.
