Al Qadim: Maybe 2nd edition's high-point in regards to setting creation. A really wonderful take on the genre they sought to emulate. The closest cousin currently available is Tales of the Caliphate Nights...but it's for True20 and I know how "D20-or-the-highway" folks can be.

Dark Sun: 2E's best kick at a "Whole Cloth From Scratch" setting. Everything about it was fresh when it debuted. Really exciting stuff at the time.

Birthright: This is the only one that really needs an edition made. Anything else on this list can be cobbled together with existing material...but not Birthright's domain management stuff.

OMFG! Why didn't I ever think to run a d20 Modern campaign set in the G.I. Joe universe? All of us fanboys from the 80s would have a blast creating our own Joes to fight along side Duke, Scarlet, and Snake Eyes whilst trouncing Cobra Commander, Destro, Dr. Mindbender, and Serpentor.

That idea seems so obvious that I am ashamed of myself for not thinking of it when d20 Modern came out.
 


Greyhawk would be my number one choice, preferably done by Paizo. My dream would be to see all the scattered Greyhawk material from Dragon/Dungeon in book form. My team of writers would be Mona, Jacobs, Gygax, Kuntz and Holian. Hey, a man can dream, right? :)

Dark Sun is a fantastic setting, but a comeback in print would absolutely require the involvement of the Athas.org people.

I'd buy a new Spelljammer book, but I'd prefer a reimagining like in the Dungeon mag.

Al-Qadim, Ravenloft, Planescape, Mystara - all good.

I've no interest at all in Maztica. I thought it was a very uninspiring line. I don't care much for Kara-Tur, either.
 

I miss Birthright. Rich Baker, Ed Stark and the core writers made an interesting twist and the standard LOTR type world. The domain rules were fun too (but could be a pain with a large domain). Perhaps it was just me, but as a long time fan of Harn fan I always wanted to know more about the domain I was running.
