I voted for Greyhawk, because I'd love to see at least one good Greyhawk hardcover for those of us who still run Greyhawk games. I don't expect a tremendous amount of support for it, but one decent hardcover for Greyhawk would really push me over the fence into playing 4E.
I wouldn't mind some sort of Kara Tur/OA type setting, but I'd like it to include different asian cultures than just Japanese with a dash of Chinese. Such a setting could be really cool with a significant section modeled after Indian and southeast asian mythology.
Other than these limited examples, I agree with some of the other posters that the brand-new edition needs a brand-new setting. If 4th edition is really going to be as different as it looks, any older settings (even Eberron) would be shoehorned into 4th edition. I'd rather have a setting designed from the ground up to account for the style of play that 4th edition is going for. I think that if the game is breaking new ground, the setting should as well.
