What real world elements show up in D&D the least?

In my experience the top 5 D&D no-shows are:

5. Food
4. Weather
3. Laundry/sword sharpening/other upkeep
2. Sex
1. Bathroom breaks

Runners up:

* Keeping track of ammunition
* Keeping track of encumberance
* Any kind of rational limit on carried equipment (I once gamed with a guy who said, at the table, with a straight face: "I sheathe my glaive and draw my bastard sword and tower shield.")

One of the campaigns I run is a one-on-one campaign with my wife...;)
 

Going to the bathroom's the only thing that's ignored even when relevant, eg PCs camped in dungeon for a week without toilet facilities.

Well, the dungeon in Scourge of the Howling Horde has a bathroom. Two of em' actually.

I'd guess the lack of toilets and going-to-the-bathroom rules probably has something do do with not giving a legion of teenage D&D players any more ammunition for toilet humor. As it is, they usually don't need any help with that.

Food. I've had characters hang on to the same week's worth of rations for 12+ levels. I know why this is so; I did play in a game where we marked off a day of rations every game day, and it made for tedious bookkeeping.

Equipment distribution on the body. Just because you have a 22 Strength doesn't mean you have room to hang two swords, a morning star, a long bow, and a half dozen daggers on your person. I run into this challenge a lot, since I tend to draw the characters in my games. As someone who goes to Ren Faires in full garb (costume), including sword, I can attest to how much encumberance a single medium weapon is, much less 2-4.

I doubt you have a 22 STR though :p :D Actually what you listed is really not that much and depending on the swords would not be that unweildy. Though far from comfortable. Just wearing a sidearm and soft body armor none of which can be seen on me is not comfortable, and I can't wait to take it off. When I get home.
