What type of Fate Point / Action Point system do you use.

If you use a system where players may spend "points" to reroll the dice or change the outcome of events, which one do you use?

A home-made one? Take one directly from a system?

I am thinking of including one in my new Iron Heroes game (RL) but I don't know which one to use. I really like the Fate points from Conan D20 but would like to hear of some peoples experience with these kinds of systems.

Has anyone ever run into big problems or do they work well for the most part?

I've started using action points as-written in d20 Modern. They're extremely useful, especially in games where there isn't quite as much magic to save characters who can't roll the numbers required by checks, saves, attacks, etc. The number of AP gained per level increase slowly over time, but the bonus doesn't. This helps prevent them from allowing over-the-top bonuses, but I think it would be cool if one could roll multiple dice and keep the best roll at higher levels. Currently only a few Advanced/Prestige Class abilities allow this, and often only with specific d20 rolls.
 

We decided to implement this in our D&D game. Homemade system.
Characters have one action point per five levels (rounding up) each session.
i.e.: a lvl 11 char has 3 action points that he might use during the session.

they add +1d6 directly to any roll (or to AC).

(we use to play pretty heroic, death is pretty much a non-risk... I would like a grittier kind of game, but this isn't my rant-thread)
