You can happily run an unbalanced game well, but doing so requires more effort on the part of the DM than otherwise.
The more unbalanced the game, the better the DM has to be.
(The Book of Exalted Deeds makes D&D unbalanced, but that's why you need mature players for it!)
D&D, as a game played by beginners through advanced players, in tournaments and at home, by millions around the world, needs more attention to balance than the homebrew system that is played by your local group.
Attention has been paid to balance in all incarnations of D&D.
