When occupying the same square . . .

Okay, let's talk about some situations when occupying the same square: Some default assumptions – this is for either 3.0 or 3.5 (ie let's draw on both), and we’re only going to consider the example with medium creatures.

Starting with striking a grappled opponent – the rule is pretty straight forward: I couldn’t find a ‘hit your ally chance’ in 3.0, though I'm sure its in there, and the chance to strike your ally in 3.5 is 25%.

However: What if two opponents are not grappling, but are occupying the same square – for example if a grapple has been broken, but no one has yet moved ? Again I can not find anything in 3.0, and I don’t have the 3.5 books at the moment. Does the 'hit you ally chance' change, disappear ?

Finally, does anyone think, either from a rule or a logical derivative from core rules, that the chance to disarm an opponent should be affected if the opponent is grappling, or is occupying the same square as an ally.

Um, no, it's not. You're thinking of Bull Rush.

There's no chance of targetting the wrong creature in a grapple with a melee attack. With a ranged attack, the grappler you hit is "determined randomly" - the percentage chance is not given, but presumably it's equal if they're the same size category.
