D&D 5E When to use Shove and Grapple

I'd like to see some way to pin grappled, prone targets.

I'd also like to see some rule of thumb for giving monsters proficiency (or whatever) with grappling when they shouldn't be mopes when it comes to grappling. E.g., I doubt your average orc is a stranger to grappling. I don't even want to do the math for how many orcs it takes to hold down a fighter with Athletics proficiency and a level of thief for Expertise.

Db, I was thinking more of a grappled target's ability to continue attacking. Would also like to know how to tie up a grappled creature.
 

they attack with disadvantage. They need to use they action to make a athletics or acrobatic test agaist the athletics of the grappler. There is many monsters with athletics prof, so they are prof in grappling.
