Yes, and yes. You can use the followup attack to make a disarm attempt, and standing does provoke an AoO.

Indeed... but whether you can trip someone with the AoO provoked by the Stand From Prone action is disputed.

There's argument that until the action is completed, the target is still prone, so tripping provides no advantage; after the AoO is finished, the Stand From Prone action resolves, and they end up standing anyway.

Except you can take a 5' step DURING another action. So you can be taking your attack and make the 5' step simultaeneously, and choose to resolve the 5' step first.

If you resolve the 5' step first, then 'immediately' has happened. If you resolve the 5' step second, you're too far away to attack with the rapier.

While I agree with you Hyp, I think the point is that you could resovle the 5' step as "during," as perhaps then it works. I don't think so.

That seems shaky. I can ready an action to attack an enemy mage when he casts, and even though my attack is resolved before the enemy mage's spell is (and hence should be when he isn't yet casting), the damage is still said to disrupt his spellcasting. Of course, this is explicitly stated. I gather that there is no similar explicit statement for Stand From Prone.

Check the reasoning given above for readying spells. The spellcasting is resolved first, yet it actually occurs during someone elses action. Certainly a readied action can interrupt most situations where the word 'immediately' is used in the SRD. Immediately isn't as immediate as you think.
