Why Fiends rarely enter the prime material plane overtly:

Except they always get traveler's tummy. Maybe its the water, maybe its the paladines, but they often end up needing to spend days in the outhouse.

Now, look at the typical dungeon floorplan. See many bathrooms on it, do you?

When you have fiendish diarrhea, you really don't want to have to wait in line at the loo behind the other wandering monsters.

No used to the Prime, Paladins and pesky Epic Heroes running around, Monsters roaming around and celestials and gods would take offense if demons would invade in hordes.

Better you deal with the demon you know next door instead than invading other people´s playground.

It's the gnomes. Really. Thought there was no purpose to having gnomes in the books, some say. But they make sure the fiends get better rates on flights to other planes of existance. The Gnomes of Trav L'ahsitee work to keep the fiends away.

Remember that they see themselves as living for eterity. Thus, they don't feel urgency to act the same way mortals (even dragons) do. This helps explain both their interest in humanity (incredible productivity, making and unmaking civilizations in the same millenia!) and their relative lack of productivity with regards to them.

The real reason for wandering monsters in a dungeon (and their bad attitude) - they are in the search for a bathroom!

I don't know what you are talking about, Jennifer Aniston, Courtney Cox, David Schwimmer, that guy from Lost in Space. They're plastered everywhere. They are completely overt.

Wait, that's FIENDS. I thought you wrote FRIENDS. Honest mistake.
