Essentially, First Blood matches are 1-flagt speed matches, but there is a catch: In order for a move to count as a hit, it must both deal more damage than their opponent's move, and deal as much damage as their opponent's tier. Fists of Fury are counted as seperate moves.
For instance, a Tier 3 Silver Sash and a Tier 3 Gang Boss are dueling. The Yakuza's move matches their Signature Style, and they use Sneaky Trick to make it match their location, for a total of 1 (base) +1 (style) +1 (location) = 3 damage.
The Sash generates a Ruby move, and uses a Poisoned Blade power that they baught for their custom item. The Sash had fought that Yakuza before, and purchased a Mark on their weapon against them. They deal 1 (base) +1 (modifier) +1 (poisoned blade) +1 (mark) = 4 damage, higher than both the Yakuza's damage and tier.
The Sash scores a hit, and wins the fight. If the Yakuza had had an unused Dirty Trick, they could have used it, and forced the Sash to generate a new move, which probably would not include a Wealth modifier. The fight would then have continued on to the next round.
In another example, two White Belts are in a match. One generates a normal move, and the other generates a move that uses Manticore, which happens to be the Day of the fight. They deal 1 (base) +1 (day) = 2 damage, higher than both the damage and tier of their adversary. They score a hit and win the fight.
