D&D General Your Character Died! Who's Fault Was It?

My last PC death was at the hands of a fellow PC at the conclusion of our game. The party was fighting alternate versions of themselves for control of a prize, and after we won we turned on each other (everyone had a different reason for wanting the prize, and it was an evil campaign). It was a blast! I came so close, and only lost because the spirit of a PC I had abandoned earlier in the campaign came back to take revenge in a critical moment of the combat.

One of the most fun games I've played in in years, and the fact that my PC died just before achieving their ultimate goal of world domination didn't harm my enjoyment one bit (or anyone else's).
