My situation:

A dead monster exploded at the same time as our local barbarian was due for a run-and-smash (which would get him out of the explosion range).

I had him and the dead monster roll opposed initiative checks, which the hero won. yay!

Whenever there are competing speed things like this, i handle just like that.

Or this comes up in EVERY SINGLE SESSION of my game:

Insane halfling paladin: I want to decapitate this guy.
Reasonable fighter: ugh.... I want to attempt to stop him from decapitating this guy.
Me: coo, roll those competing initiative checks.
