This'll be a bit repetitive with above posts, but . . .
Blackmoor is its own campaign setting, although as mentioned it is a region within a larger world (but most campaign settings are). It was NOT incorporated into Greyhawk, but a region of Greyhawk was named Blackmoor. It WAS incorporated into Mystara, but not so much as a separate region but rather as an important region in Mystara's distant past. It was one of the original D&D campaign settings and was published second only to Greyhawk itself. The campaign's creator, Dave Arneson, who also co-created D&D, did retain some/all (not sure) of the rights and authorized a third-party d20 version of the setting a few years back. Did Blackmoor have a special name for the planet it was set on? I don't think so, but I'm not sure . . .
Council of Wyrms was NOT a fully independent campaign world, but was designed to be easily incorporated into your home campaign as a large archipelago of islands where all the dragons hung out. I do not believe a name was ever given for a planet, as the setting was for any campaign.
Above posters have Jakandor nailed. Like Council of Wyrms, it was a "micro" setting that could be incorporated into your home campaign, but was also detailed and big enough to run independent games totally within the setting. Interesting factoid, the Jakandor series was originally going to be called "Steel & Bone", which I thought was a very cool name for a campaign setting!
Ghostwalk actually was a complete setting of its own. Although really it was presented as another "micro" setting, just with notes on the surrounding lands. In fact, it was a lot like Monte Cook's (the author) Ptolus product, where the book focuses on a city that could be easily incorporated into any home setting, but included notes on the surrounding campaign world if you wanted to use it.
Edit: Oops. Forgot the OP! I'm pretty sure that NONE of the mentioned settings had names given to the planet it was set upon, and of course as mentioned above, several of the settings were meant to be placed upon the planet of YOUR campaign setting.
