If they’re young, a level of exhaustion. If they’re middle-aged, 1d4 levels of exhaustion and a death save once per day.

Oh, and Cure Disease does nothing.

Rules wise? If the character is female, no real change - you can continue adventuring as effectively as before. If the character is male then it's straight up Death Saving throws every round until the brave hero finally bites it.

In keeping with 5E design philosophy, an illness is either such a hindrance that it causes disadvantage, or it's so minor that it's not worth tracking. I'm inclined to go with the latter, except on scent-based Perception checks.

Close. If the character is male they must loudly proclaim that they are dying, doing 1d4 - charisma modifier* irritation damage to everyone within hearing distance.

*Because some guys, based on what I've seen of women's reactions, are just too cute when they're sick.
 

A more serious answer is a level of exhaustion, with a daily saving throw. Fail 3 saving throws and you've contracted pneumonia and gain another level of exhaustion. Repeat until dead or better.
