Considering the year on year growth of the game the issue doesn't seem to be hurting. As always, there's room for improvement. I think expunging the concept of good and evil from the game is a mistake.
Hurting sales? Hurting by embedding lazy thinking with regards different groups in society? Impossible to measure.
P.S. study after study has never shown violence in fiction leads to violence in real life.
It's possible to have discriminatory thinking without actual violence.

Anyway, it doesn't matter what is true, it only matters what people believe.

So, these cultists who join your Doomsday cult all do so of their own free will, of sound mind and body, fully aware of the evil nature of the power they serve? None of them have been brainwashed, lied to, raised by the cult, loners looking for acceptance, love or family, or forced or coerced to serve the cult against thier will?

Just making sure these nameless cultists are true believers before I cut down swaths of them with steel and spell...

You willfully join the Infernal Horde. The Acolytes of Ends use trials to be sure potential cultists are worthy.

Taking any of them out of it is a very high Religion check.

I PCs kill someone outside self defense, and feel no remorse them they are EVIL. especially if they take pleasure from it. Or maybe they are just sociopaths and simply do not care either way, so maybe they can be neutral(leaning towards evil).

Or they can be good in some aspects and evil in other.

If killing creatures outside of self defense is evil, then every soldier who has fought in war is evil. It also makes D&D or any heroic fiction problematic. There are always nameless guards that get in the way of stopping the doomsday device. There will always be people on the Death Star.

That's the point. I created a fantasy fiction completely separate from real life.

So if someone claims I created something resembling real life and its offensive, I will threaten to slap them with my DMG to knock the sense back into them.

If someone says I've done something offensive with the fictional setting I'm running D&D in, I'll ask for details. If they're being daft, I'll hit them with a Pathfinder Core Rulebook or something. If they're right, I'll see if I can fix it.

killing in war is usually abstract version of self defense. Unless soldiers commit additional crimes(torture, rape, execution of POWs, knowingly targeting unarmed civilians, etc...)

Self-defense can reasonably be presumed to include defending your home, or your loved ones.
