Best new creature, in your opinion!
I must be in an undead mood today, because I think that I will answer: The Graveyard Revenant.

(I had to go through the whole book again to answer your question, plus double-check that I'm not just forgetting that anything already existed)

How many types of Cultists are there?
Seven.

Since @Upper_Krust hasn't jumped on yet, I will ask: what is the bite damage for the tarrasque?
4d12+10 plus grapple.

Pretty sure I already know the answer to this, but hope springs eternal.
Are there any templates?
I don't think so.

Is the Tough NPC identical stat-wise to the Thug in 2014?
Looks like it is.

How useful are the Indexes?
Haven't used them, but look useful.
and how flexible are the NPCs
They're very similar to how they were before. I'll probably add a basic form of "species customization" to them. Though with how I do things, that will probably amount to "remember approximately what a species can do and use it with the NPC block if it comes up. Like, say, having a Dragonborn NPC breathe or giving a Dwarf NPC poison resistance, or that sort of thing.

How many entries under "D" on the table of contents?
24. And 7 of them start with "Death".

Do you use mayonnaise or butter for your grilled cheese?
Both. Butter on the outside and mayo on the inside, though spread thin so as not to have the cheese slide out.
Are there multiple Flumph entries? Dire flumph? Flumph of annihilation?
No, just one. Missed opportunity, I say!
What is a monster, anyway?
My answer to that, as of yesterday, is entirely political and would get me kicked off ENWorld.
