Is there no longer a way to set a visual theme other than the garish yellow on black? I used to have a grey/black theme that was much easier to read.

Of course it will. Every edition of D&D starts off simple and innovative, then power creep sells books until it's completely broken and the process begins again. The only new thing for 3rd edition is the relatively short time for a version turn-around compared to its predecessor.

You haven't missed anything, it's D&D, there are lots of things that aren't realistic, including the fireball headed up the chimney in the first place. You can always house rule that you're denied a reflex save in such circumstances if that works better for your group.

Why are we even looking for rules? Common sense should be sufficient here--you can't trip something that can't fall down.
