Suggestions for enhancing Isle of Dread?

I did a direct conversion of the original, didn't use the next one. There is a sort of plot: find the center of the Island and the ancient things that lie there. Even some gentle nudges can get the players heading in that direction.

But I would not shy away from the broad scope, its not just dinosaurs. There are pirates, cat-people, natives to trade with, and more. I would skim it, maybe pick 5 or 6 of the things you find most interesting, and think of some hooks or hints to get the PCs engaged.

Infestation. A living creature injured by a botfly swarm’s attack must
make a DC14 Constitution saving throw or be infested with the 1d8
botfly larvae.
Swarm. The swarm can occupy another creature’s space and vice versa,
and the swarm can move through any opening large enough for a Tiny
insect. The swarm can’t regain hit point or gain temporary hit points.

Actions
Bites. Melee Weapon Attack: +3 to hit, reach 0ft., one target in the
swarm’s space. Hit: 10 (4d4) piercing damage, or 5 (2d4) piercing damage if
the swarm has half of its hit points or fewer.

Botfly Larvae
A creature infected with the botfly larvae do not immediately recognize the fact that they are infested with a parasitic insect. One day after being infected, the larvae hatch and begin gorging on the flesh around the original wounds; each successful instance of damage counts as a separate point of infestation.

At the points of infestation, the flesh becomes puffy, red, and extremely painful, causing the infected creature to be poisoned. While poisoned, the creature only regains half as many hit points from spending hit dice.

A character can make a DC14 Wisdom (Medicine) check to remove the botfly larvae one at a time, each attempt dealing 1d8 slashing damage.
