    I would say I do both, as I usually plan out an adventure but will throw it to the wind, as the players go in different directions. And usually my planned adventures are very light on info, so I have to adlib on the spot often anyways.

    I would say yes, as the Iron Bands, literally wrap the character up in them. Its not a cage where they can dodge the strike. The best you could do is roll around, and that could be easily handled by the person making a coup de grace.
