I seem to have hit a nerve. I apologize for making a reference to 4E; it won't happen again.
The problem is not that you mentioned 4e, but that you (I assume unintentionally) misrepresented it. Healing surges filled a very specific design role in 4e that hit dice only superficially imitate and the “healing surge” rule in the 5e DMG bears almost no resemblance to.

EDIT: it appears I have fallen victim to thread necromancy.

I used that variant rule in the last game I DM'd some time ago. Mostly because the party was composed of a Ranger and a Warlock. So, since there were no healers in the party, I figured that would make up for it somewhat.

Of the sessions, IIRC, only the Ranger used it when he got low on HP in one. I think he regained a decent amount of HP back. Most of the duo's HP recovery was due to short/long rest after the major events of the sessions. Then the Warlock player moved outta state and now the campaign is somewhat on hold/hiatus.
