D&D 5E 5E PC stat block?

So has anyone seen some sort of formal layout for a PC stat block? I would like to present a PC in a brief form without using the entire character sheet.

That is an example they made of one of the book characters. She's built like a 12th level warlock and has all those things. But it's still a lot of space. You might be able to base something on it.

Remember NPC's are not PC's. So if you are looking for a way to represent an NPC and not a PC, use the NPC rules in the DMG.

We need most of the things that we'd need in any other edition, and I think that the bonds ideas and flaws are important enough in this edition they should be included. The ability scores are more important so lets try something
