A poor death for a warrior

How about getting shot to death by a clown with a sniper rifle?

No, I mean a clown, as in makeup, floppy shoes, red nose, orange hair, and all. :)

Ever notice how these miscalculations increase the amount of loot the mage gets?
 

The halfling rogue that was grappled to death by a carpet is still one of my favorite :)

There was also my 3rd level monk that got killed by getting hit with a crit from a dire wolf. Went from something like 27 to -12. The rest of the party killed it in the same round :(

normal dire wolves usually only do 1d8+10 [crit max 32], unless it advanced to huge or had good buffing.
 

How about jumping into a sphere of annihilation, thinking it was a gate -- and having the rest of the party line up to follow you "through"?

Or getting knocked unconscious when a door falls on you, and dying when the rest of the party walks over it to see where you are?

Or the time I tried to slap an insane beholder to snap it out of its delusion?

Or casting lightning bolt in the darkness in a paranoid frenzy, just because you see something looming before you -- and the bolt bounces off the stone statue and knocks you dead?

Oh yeah. All kinds of fun stories.

Had it not been for Hero Points, I'd have had plenty more PCs dead at my hands... such as:

1) The paladin fighting a Reptillian Troll, which I described as lizardman-looking fellow that was over 8 feet tall, and very lanky. After getting injured, the paladin takes a five-foot step back, thinks, "I'll be safe here," and uses a wand of cure light wounds. The claw attack would have killed him, but he was saved by a Hero Point. The player looked up at me and said, "Why didn't you tell me he had reach?" Er... yeah... because that's how I'm going to describe monsters...

2) After a nice, long adventure, the whip-using rogue walks into a bar to cause trouble. Little did he know some enemies were sitting there, waiting just for him. So the first punch he recevied was something like 15 points of damage, but he was at 1 hit point anyways! He looks up and me sheepishly and says, "Can I use a Hero Point to say I used the Wand of Cure Light Wounds before coming in?"
