A lot of people weren't reading the part about 5th level, only PHB.
For a tank, you have 3 choices in the PHB: Barbarian, Fighter, and Paladin.
You can possibly also multi-class.
I'd recommend Fighter if you like having lots of feats.
Barbarian if you just want to rage and squash stuff.
Paladin if you are a lawful good type and like making saving throws.
For feats, get Power Attack, Cleave, and Great Cleave. Those should be your priority at fifth level. Other feats are nice to have.
For race: dwarf or half-orc (human comes in distant third). Other races are nice, but don't have the combat bonuses of those two.
half-orc gets +2 strength and nightvision
dwarf gets +4 ac vs. giants, and +2 con
human gets bonus feat
for stats, maximize str and con. dex is nice, but distant third. paladins need chr also.
