Is there a way to stop this?

I think this is very well thought out. It is very powerful and at first glance the +4 to CR looks appropriate but maybe to high. Maybe +3 if 3 HD or less? A pack of forsaken orcs is going to be easily sispatched by 5th level characters.

I cant help but thinking of Bunnicula when I read this, kinda scary.
 

I changed the CR adjustment to +3 ... I think I'll keep it there for a while.

I also redid the Create Forsaken special attack so it makes more sense

Create Forsaken (Su): Any creature that has had their blood drained by a Forsaken suffers from the deadly and partially divine disease that they spread.

The incubation time of this disease is one day. The DC to resist this disease is 21. Evil creatures automatically fail their saving throw. Each day that a character fails their saving throw they suffers 1d3 points of permanent Constitution damage. Any evilly aligned humanoid who dies from this disease becomes a Forsaken within 1d3 days.
