PG-13 for innuendo and sexual situations (on the players' parts), language (both players and NPCs are fairly foul-mouthed) and violence (graphic descriptions are my forte). Oh, and "epic battle sequences and scary images", whatever that means.

Demiurge out.
 

I'm with Thanee (and a lot of others, it would seem ;)) - somewhere between R and NC-17 (when things get a little fiendish). But as always, the campaign is strongly based on the players' ability to handle subjects like violence, drugs etc. pp.

They're not rated at all. I wouldn't want them to be overrated. :)

But more seriously, there's a lot of graphic violence, like in all D&D campaigns. Sex scenes are just alluded to, when they happen -- after all, they are not life-or-death situations (and I wouldn't want to play or run a scenario where they would be), so there's no need to focus on it.

Grim things and mature themes abound, though.

I keep most of my games at the PG-13 level. About 75% of all the gamers I GM for are not mature enough to handle anything beyond alcohol and violence. I keep all of the other stuff to a minimum, the bare minimum. There is only one game that I am running that is utilizing a lot of the material from Libris Mortis, The Book of Vile Deeds and Fiend Folio. That group is only rated R though. The players are all mature enough to handle almost anything but my imagination knows no bounds, so I try to keep my truly vile ideas to a minimum.

R. Graphic violence, open discussion of gender/sex/sexuality (it's an Eberron campaign, and as I pointed out to my players, when you have characters who can change gender at will and others who have no physical gender, it's something to think about), drugs, nudity, etc. Of course, I also don't think any of the above is intrinsically "mature", but that's just me :)

I don't have sexual encounters played out at the table, not because I'm uncomfortable with it, but partly because it would be a scene where only one or two of the players could participate (haven't had reason for an orgy yet :D), and partly because I think the players would.
 

D&D games I run are probably rated R for violence and 'adult situations.'

The Mutants & Masterminds games I run is closer to NC-17, but that is likely due to the really nasty villains and some of their naughty female underlings.
 

My games run from G all the way up to NC-17, depending on the content of story and plot, nad nature of the game (D&D or Vampire).

Though I don't take the moral high ground, I would be careful around children and younger teens, though once they get to about 13 they know about sex and drugs and torture (remember your own childhood... *Nods*) so I don't feel a need to be concerned there.
