best form for a 5th level druid?

Cheetah. Trip with all three attacks.

If you have time to buff up before combat, cast Bull's Strength for that extra trippin' goodness And the usual Greater Magic Fang, Barkskin and Cat's Grace. If your campaign allows it, Thornskin (Complete Arcane) is a good melee buff spell, too, as is Vigor (Complete Divine).

At level 6, you can take Natural Spell, but at 5th, you gotta cast spells before you change form -- a time-consuming way of entering combat.
