Can't sneak attack in dungeons?

These rules wouldn't just apply to rogues; if you think that sneak attacks wouldn't be possible in a given combat, then each and every attack made in that combat will be suffering from that 10% miss chance.

I'm usually pretty liberal about letting characters in melee see each other, mainly because it's a headache to keep track of subtle lighting effects all the time. For that same reason, I wouldn't worry too much about concealment due to darkness as long as there was some form of light around.

Normally, the Blur spell makes you immune to sneak attack precisely because it gives concealment.
 

A target with concealment cannot be sneak attacked - this is true. However, concealment is not exactly the easiest thing in the world to get - particularly concealment that works within 30 feet.

Most of the time - these concealment circumstances would function both ways. Fog and smoke block everyone's vision - not just that of the rogue. You would be making the percentile roll for every blow swung in the combat.

Short of magical concealment methods or getting caught in a night ambush - I don't see how this could come into play.
