Hit point damage does not necessarily represent full contact between the weapon and the target. The giant can "do damage" without having his club actually connect - all he has to do is get close enough so that the pixie must strain like mad to keep from being made into a home run :)
It's like talking to a wall sometimes, isn't it?
 

I'm also one of those that plays with the use a little common sense tactic. Many fights with my players they have had to make balance rolls and such. One Ranger in particular loves leaping up on benches and tables to make attacks over the heads of party members and enemies. He falls on his ass a lot but loves it.

In a recent game, the party was camped in a guard room in the dungeon. Their decision on how to guard their position through the night? The above-mentioned Ranger will sleep sitting up with his back against the door :D . During the night, the captain (a hobgoblin) comes to check on his guards and see if they are sleeping on duty, pushes against the door and it doesn't open due to the Ranger leaning against it. This wakes up the Ranger, who says he's standing up and drawing his weapon. In the meantime, the captain decides he's bursting the door open to surprise his sleeping guards and slams against it. My judgement was the door slammed open, catching the rising Ranger in the butt and hurling him off balance across the small room, taking 3 points of damage (half of what the captain did to the door in opening it).

No rules for that that I'm aware of, just a quick decision made on the spot. That's my job, it's why I'm sitting behind the screen at the end of the table. Otherwise, we all might as well be playing Everquest.
