Sorry what are you saying? Per encounter powers recharge after 5 minutes rest outside combat. So If you use one and then get attacked within 4 minutes you won't have that /e for that encounter because it hasn't recharged. What is wrong with that? It is just the rules.

An encounter could last anything between 5 rounds and 5 minutes. You must have a short interruption of combat. If you end a combat one minute ago and face a new encounter, you can use your encounter power again.

Heal skill allows you to, with a DC 15 check, activate someone else's healing surge.

I'm not 100% sure if it triggers your Second Wind, if it's limited to once per encounter or... what.

This is completely wrong. Encounter powers are not restored until you take a 'short rest', which is a rest of at least 5 minutes.
 

I'm mainly suggesting the application for Heal Skill for Clerics to fill in until we see full description of it and how the Cleric handles out of combat healing. My suggestion is making Heal for the Cleric the same as Rogue using his skills related to traps or Ranger for Tracking that others without said classes cannot, if only until we discover the out of combat healing rules.

Most likely there will be rituals Clerics have access too that will cover it, but take longer to work out than the combat healing spells.

Even of you face a new encounter? It is once/encounter, I might be wrong with my interpretation, though. But, if so, it's going to be HR at my table.

Well to me a short interruption of combat is a short rest, my first house rule for 4E.

EDIT: But I won't allow my PCs to use a healing surge without a full 5 minute rest, just for encounter powers if there's at least a full minute break between both encounters.

This would not be a new encounter, if you faced more monsters without the "short rest" timework in between it's a continuation of the same encounter you just get out of....also your DM is a jerk....or you are definately in the wrong place at the wrong time. So no encounter ability recharge without the 5 mins.

I'll need no houserule as I DM and thus I have complete control on when monsters come upon the characters. If that is unrealistic that's because this is a game of imagination.
