The creature running away can't withdraw and then ready an action to run away in the same round. It has to make a choice.

If it chooses withdraw, then you can charge it.
It it chooses move and ready, then you can take the AoO.
 



The arenas are consider the prime material plane. This makes most outsiders extraplanar and thus subject to dismissal. It also makes them subject to banishment, but based on a peculiar wording, only banishment from folks native to the prime material. The hollyphant, for instance, is not from the prime material and thus its very useful banishment ability is technically useless in the arena.

1. The change is for fights in ToC only, not the main battle. Your general is built on the right assumption.

2. Rule changes are being discussed right now. None have been adopted, and the earliest they would be adopted is when every alignment finishes fighting Neutral in the first round.

3. See the answer to 2.

4. Once all the alignments fight Neutral, they start fighting each other with what they have left over from the first round.
