None of the above, actually. He gets 3 ranks at 4th level, because his Int was 13 for the duration of the previous level. At 5th level and up, he will get 4 skill ranks.

This is found in the PHB on page 145 - note that you assign skill points before you raise an ability.

This is in direct conflict with the example on page 10, but as it goes with the "Intelligence increase must be near-constant over the previous level" guideline it is the correct way to interpret things.
