Dead Saving Throw

I was also going to say "not rare" but decided, I don't really know what "rare" might mean in this context.

Looking at my paragon level home game, there a total of zero PC's with any kind of bonus to death saving throws. The only kind of bonus I can think of (for any kind of save) is the dwarf's +5 vs poison.

In a paragon LFR group I'm playing in, I'm stacking the horrid cube and resilient focus for +4 to all saves, and I'm not the only one, so save bonuses seem like pretty common place.

In an epic game with a paladin that has Hero's Poise that gives everyone a +8 or +9 to saves, saving throws can become trivial to pass.

Yup, one of the reasons relatively poor defenses often don't matter much at epic. The enemy can lard conditions all over you all they want, heck they can auto-hit with them. Any decent epic PC will be saving before his turn starts and doing it with a large bonus. Many of them don't even need a leader around to do it (Warden, forget it, you might as well have "all conditions end at the start of your turn").
 

In our party, my character has the Resilient Focus feat giving +2 to all saves, and our dwarven cleric has a Paragon Path class feature which grants nearby allies +5 to death saves. My warlord also has a utility power that grants him and all allies a +5 power bonus to saves after an ally succeeds on a save.

If all those happened to be in play, I could potentially hit 20 on a roll of 8 or more.

Yep - the sun domain warpriest in our party (who grants a +2 to death saving throws off the bat) has the Harbinger of Rebirth feat. Our party regains consciousness on rolling a 13+. "Popping up" after spending a round unconscious has become so commonplace that our dwarf paladin took the Gold Dwarf Pride feat recently and deliberately aims to be unconscious by the second or third round in order to gain the benefits from the the feat (+1 AC, +1 to attack rolls).

A Starlock's pact gets him a "+1 to next d20 roll"- +2 if he burned a feat- and he can accumulate as many bonuses as he had Cursed foes fall in the prior round.

So, for example, if he has to make a death save after 5 Cursed foes were dropped, he could have a bonus of +5-10 to that roll- or any other D20 roll- just by himself.
