Our DM decided that the enemies know exactly what is going on, but he plays them appropriately - zombies don't really strategize, for instance. In return, he also keeps us aware of enemy specials, at least most of the time.
I think the RAW would be that the enemy knows he's marked but not the exact consequences apart from the -2 to attacking anyone else.
In our games CC doesn't come into play regularly, but when it does it's pretty cool - the enemy may be in a lose-lose situation where he gets penalized no matter what he does.
