The current D&D group (mostly because we've not played enough to worry about it) isn't named. The d20 Call of Cthulhu are the British Defence Initiative (BDI). The last major D&D campaign I ran the group called themselves the Jesters of Destiny (after the band). There was also once the Committee for Public Safety. Not every group or campaign has named themselves, but those are the ones I remember.
 

Typically when my party has been together for a long time, we'll pick up a name.

My current camaign (now 3 years long) the name came up after a particularly... curious encounter they had.
 

Names kind of happen once enough has gone on for there to be a reputation or a general party theme.
The 2nd ed group I ran became the Company of Crazed Companions.
In Eathdawn we were The Unruly Mob.
My current group are the Freewill Adventurers.

Oddly enough, a vein of chaos and insanity seems the norm. :\
 

In my short-lived Eberron campaign my players ran a mercenary group in Sharn which they named the Riskbreakers (after the organization in Vagrant Story). However, that's rather infrequent and is mainly a result of how I like to run campaigns -- by making sure the party has a sense of comradre even before the camp starts.
 

We had a group called "The Wings of the Hawk." We play those PC's off and on. We all share the DMing duties in my group and none of our other groups of PC's have had names.
