The answer is twofold: 1) so the mammals won't mistake a Dragonborn's gender, and 2) secret compartments no mammal would search except in the rarest of cases.

Dragonboobs are needed so that the nipple clamp of exquisite pain can be used. I thought this info was common knowledge.

I suppose a codpiece of impaling could be invented to go with the clamp. Then everything about dragonborn gender would be explained.
