It's a skill rather than a Fort-defense attack to keep it constitution rather than strength.

And I think being able to be trained in it makes sense; something as simple as holding your breath for a long time can be increased by practice. Surviving in difficult environments, even more so.
 



Diseases are really scary now, and endurance works against them. That's something.

Though I'd not be surprised if most groups just pick up a Keoughtem's Ointment and move on...
