My current high AC character has often deliberately provoked the AoO for other characters to withdraw to safety. Also one time he took an AoO on a mook thereby allowing the horde to flow past him to the 2nd liners.

As a result, I think that there can be an overinflated fear of AoO and also just because you can doesn't mean you should take it. All in all, the AoO part of the game can be pretty fun indeed.
 

Had another PC die from an AoO.

Even better, the character died from rolling a 1 on the save against massive damage resulting from the AoO, which was triggered by a charge against the BBEG, which most likely would have fallen, if the charge would have hit; and all that, after that very same character already died once in that very same combat, also by rolling a 1 on the save. Tough luck for sure! :p

I've willingly taken AoOs many times, but never have I died. That's one of the aspects I like most about 3E government nowadays. The tactical decision that go into movement around the enemies, taking AoOs so the wizard might cast his spell uninterrupted or that the rogue might better position into a flanking situation.

And all that without a battle map and minis :p

Yes. My PCs have died in numerous ways, including the AoO.
 

As yet, no PC in any of our active campaigns have died from AoOs.

NPCs? Only a few- but several have suffered key wounds.

I couldn't vote. I've never seen a non-newbie player's character draw an AoO. Ever. Without something unusual, like an invisible NPC who happens to be in the right area.

I've seen a few NPCs die by drawing AoOs though. A few times in a Modern campaign I was running, and once my PC killed an NPC who used a ranged attack while standing 5 feet in front of me. (I critted on the AoO, killing him.)

Ironically these NPCs always seem to die when drawing AoOs; even if they drew one at full health, Murphy's Law dictates they'll get critted, fail a save vs massive damage, and die.

As a player, I don't believe I've ever drawn an AoO (barring unusual circumstances like an invisible PC). Arguments erupt if a PC accidentally draws an AoO, and that only happens if there's no "graphical interface" like a battlemat.
 


Our groups tend to draw alot of attacks of opportunity for trying to do things like grapple, disarm, sunder, or bullrush without the appropriate feats. In one campaign we almost had a TPK because two of the five PCs were unconscious, and one had been knocked off the balcony and had a ways to go to get up to us, so the remaining player on the balcony decided to follow the mook's lead and try to bullrush him off the balcony. The attack of opportunity knocked that PC out. I can't remember how the party ended up getting out of that one... It probably had something to do with the fifth PC, who didn't used to do much in combat but ready for a charge.. ;)
