Fang of Loth

A 9th level Fang of Loth (from Sound and Silence) has two extra arms and a bite attack.

I'm a tad confused about how many sneak attacks the character could deliver in one round.

Normal iterative attacks plus one melee attack for a weapon in each extra claw plus one bite.

Yep. Or, he can use a weapon in the claws. If he has Two-Weapon Fighting and/or Improved Two-Weapon Fighting, he can't use them and the extra claw attacks at the same time, though he could take advantage of the bite.

Since there's an Epic feat for Perfect Multiweapon Fighting and Perfect Two-Weapon Fighting, you might be able to talk your DM into Improved Multiweapon Fighting and Greater Multiweapon Fighting, to let you get second and third attacks with your other three hands.
