Favorite Editorial Mishaps?

What are some of your favorite editorial mishaps? It could be something you misspelled or simply didn't fix typing, or something you saw published that was funny, or just downright bad (The Conan D20 origional Spellbook misspell comes to mind in that case).

For example: I used to make character sheets using text files, and had made one for D6 Star Wars a long time ago. And I never noticed that there was an entry for Focre Sensitive untill almost 2 years later (Strangely, the first time anyone else ever mentioned it).

There's a spell in Races of Eberron called Furnace Within. Here are the components:

Components: V, S, DF, dwarf

So a dwarf's a component now, huh? I'm picturing some guy carrying around this dwarf in a sack just to cast this spell? And is the dwarf "used up" after the spell is gone?

And yes, I know you have to actually be a dwarf to cast it, but it still looks really funny and, honestly, I can see someone using a dwarf as a component.

One of the earliest games produced by Mayfair Games was a ziplocked boardgame called Translyvania.
I don't think the misspelling of the title hurt it's sales any, the game was a bit of a stinker.

In one of the non-WotC 3.0 monster books there's mention of a legendary slaad leader that takes the form of a "great, black salad". Unfortunately, I don't have the book to hand to check the details.

I recal in a previous edition of D&D, there was a product in which the word "mage" was auto-edited to "wizard." Well, there was at least one occurence of the word "damage" that got changed to "dawizard."

That would be a wizard from Chicago. Da Wizard!

Not a published typo (that I know of), but I remember reading somewhere at some point someone who mystyped Beastmaster and added an r to it after the B.

Not really a typo but we were playing with this kid and we asked him to describe his character for us, so he is reading right off the top of his character sheet, how tall, looks, alignment stuff like that. Anyway he gets to sex and seeing size right next to it he says, "My sex is medium." We all looked around and were trying to figure out exactly how that would translate to real life.
