I introduced the 4E eye of fear and flame--which development renamed to the "flameharrow," I believe to avoid confusion with the eye of flame beholder--in "The Ashen Covenant" article.

I've been able to sneak a couple of Fiend Folio monsters into adventures, too. The norker shows up in H2, along with the needleman reconcepted as the arborean.
A few other creatures started out as mechanics, then received major flavor makeovers. For instance, the qullan became the foulspawn berserker. The designed version floated around for a bit and then found a home in the foulspawn entry. I'd still like to do a version that keeps the original name and flavor.
I'd love to do the spawn of Kyuss and bullywugs. I also posted my take on the adherer and the blindheim over on my WotC blog. If I knew how to work the Intertubes better, I'd format them in HTML for easier posting.
The "living wall" appeared in White Dwarf 7 (June/July 1978). It was renamed "stunjelly" and included in the first edition Fiend Folio.hmm, not sure where the livng wall debuted, but pretty sure it was not the 'Folio.
