Another alternative is the Critical Fumble Deck, made by Paizo (of Dragon and Dungeon magazines, and Pathfinder, fame.) I really like it so far. Fun, and it works. Their Critical Hit Deck is likewise excellent.

Otherwise, there is also Torn Asunder: Critical Hits, from Bastion Press. It might now be printed by DragonWing Games, but I'm not sure. This book covers called shots, critical effects, critical fumbles (these, only minimally,) and plenty of other things too. Recommended.

If you don't want to buy anything, my apologies, and best of luck finding and/or making something that suits. Just thought they might be products you're not aware of.
