I just created this, and hope others might find it useful as well.
It's a sheet for players to keep track of their character's state at the end of each session. Specifically, it's to keep track of:
Current HP
Remaining surges
Expended daily powers (this assumes you never end a session in the middle of an encounter)
XP gained this session
Total XP
It's two pages, meant to be printed front and back. Page 1 has spots for character and player names, and 12 records; page 2 has 13 records. If you prefer one or the other, you can just print that page.
