After some deliberation, I've decided that Gestalt characters may combine a single class level with each Monster HD, but not with their LA.
I am considering not allowing this for Dragon or Outsider hit dice, however, since they stand very well on their own.
edit: Actually, the idea just came to me to count Dragon and Outsider hit dice as Prestige Classes for the purposes of Gestalt characters-- you can only add base classes to them, as opposed to other Monster HD, which allow you to add either a base class or a Prestige Class.
