Under which conditions is the damage dealt by this invoker power?

If the target is subjected to forced movement, does he take damage? What if he teleports?

Does using a move action to stand up from being prone trigger the damage? What if the target square is occupied and thus he is allowed to shift?

If an character makes a move action he takes damage..

Standing up is a move action, Shifting also..

Teleportation by itself is no move action, but look at Fey Step (Eladrin racial) and it say's it costs you a move action. (DM should clarify this for further gaming)

Forced Movement also gives damage..
