As far as Star Wars is concerned...
The absolute greatest Star Wars villian is Jilead Pelleaon. For the simple fact that he's survived in the Star Wars Extended Universe from well before the Return of the Jedi right up through the New Jedi Order. He started off as a Captain, and he's still alive and kicking as a Grand Admiral.
He's the Wedge Antilles of the Empire.
Ah, but he ended the war with the Republic and has been quite content to just occupy twelve sectors near Unknown Space. Pelleaon's hardly a hero, but he's barely a villain.

I have to agree with Demiurge, Pellaeon made peace with his so called enemies (and make no mistake it's what he wanted). Palpatine was pretty succesful seeing as he effectively ran the entire galaxy for about thirty six years.

Lavos. From Chrono Trigger. If you've played the game, you will know why. Let's just say that he appears emerging from the middle of a continent, lava rains over the world, and that's not the really scary bit you discover later on...

For real-world villains, how about Ilich Ramirez Sanchez? A man who spent three decades running around blowing stuff up, also known as Carlos the Jackal. Distinguished from other famous terrorists by his ability to actually do stuff in person and not provoke entire wars. Captured in 1994 by French agents, who themselves aren't too bad at blowing other people's stuff up, but that's another story.
 


If you want real-world villains, it's hard to beat Gavrilo Princip. He's the man who assassinated Archduke Franz Ferdinand, whose death led inexorably to WWI, which led to WWII, etc. Uncounted millions died because of his action.

I'm sure he didn't mean to start a war, but good intentions and the road to hell and whatnot.
 

Lord Soth alone is best! I know he is something of a Vader ripoff but I liked him better anyway. He would have more presence than Vader if put to the screen correctly. IMHO.
